At Community Connections of Clearfield/Jefferson Counties (CCC-J), our Mental Health Department is dedicated to fostering a community where exceptional support is readily available. We envision a future where adults, older adults, and transition-age youth facing serious mental illness and co-occurring disorders can achieve recovery, resiliency, and self-determination.
Our Services
We are dedicated to providing a wide range of mental health services designed to support individuals and families in Clearfield and Jefferson Counties. Our comprehensive programs address diverse needs, including emergency support, housing assistance, specialized children's services, , and community integration, all aimed at fostering recovery and well-being.
Crisis and Emergency Services
Crisis and emergency mental health services are available through our county MH Crisis line at (800) 341-5040 or the National Suicide and Crisis Helpline at 988.
BASE Service Unit
The Base Service Units provides information and referral on the county mental health system to anyone, completes intakes for individuals that are eligible for county funded services, authorizes county funded services, and provides Administrative Case Management to individuals as needed. The Base Service Unit offers appointments or walk-in services in DuBois, Clearfield and Brookville or in the community if circumstances require. Service Access and Management, Inc. Community Connections of Clearfield Jefferson Counties, does not provide mental health services directly, rather we contract with providers that specialize in meeting the mental health needs of our residents. The Base Service Unit (above) can provide information on our contracted service providers including descriptions of the services they offer, where they are located, and how to contact them. Residents with insurance should reach out to their insurance member services to find programs available through them. Specifically, residents with Medicaid can reach out to:

Housing Program
Community Connections of Clearfield Jefferson Counties, contracts a Housing Specialist and Aid to administer our U.S. Department of Housing and Urban Development (HUD) rental assistance program for chronically homeless individuals with a mental illness and other housing supports as they are available. Call our Housing Specialist at (814) 371-5100 ext. 315 or Housing Aid at (814) 371-5100 ext. 314 for information on currently available housing/homeless support for mentally ill individuals in Clearfield or Jefferson Counties. Unfortunately, Community Connections of Clearfield Jefferson Counties does not offer emergency housing/homeless services. Individuals or families with an urgent housing need should call 211 or other local services.
Children Services
Navigating the mental health system can be challenging for families with children; so, in addition to providing a Children’s Administrative Case Manager at the Base Service Unit; Community Connections of Clearfield Jefferson Counties employs two Program Specialists that can help families identify their needs and find appropriate services to meet them.

The Student Assistance Program (SAP) is a service designed to help school personnel identify behavioral concerns among their students that pose a barrier to school success. The primary goal of SAP is to help students overcome these barriers for them to be successful in school. Community Connections of Clearfield Jefferson County employs a SAP Liaison that consults with and provides behavioral health screens for the following public schools/districts: DuBois Area High and Elementary Schools, Glendale, Philipsburg, Clearfield County Career and Technology Center (CCCTC), Brockway, Punxsutawney, and Curwensville. The Clearfield Jefferson Drug and Alcohol Commission provides SAP Liaison services for the other public schools in Clearfield and Jefferson Counties. Families should reach out to their school for information on their school’s specific SAP process. The Child and Adolescent Social Service Program (CASSP) is there to help families meet complex, often multi-system needs, for their children that have tried multiple services but still struggle to stay safe in the home or community. Community Connections of Clearfield Jefferson Counties employs a CASSP Coordinator that brings together a team of experts from all systems to help families explore all available services to meet their child's needs. A CASSP meeting must be held prior to any child being placed in a residential mental health program. Families, mental health providers, schools, County Children, Youth, and Family Services, probation, or others involved in supporting children may request a CASSP meeting. Community Support Program
Community Connections of Clearfield Jefferson Counties contracts a part-time Community Support Program (CSP) Ambassador to support individuals with mental illness, their families, and behavioral health professionals in Clearfield and Jefferson Counties. The program aims to promote successful and fulfilling lives for our residents with mental illness through education, support, and advocacy. Our CSP Program regularly provides educational and anti-stigma events for our residents with mental illness and their supporters. The CSP Ambassador also amplifies the needs and concerns of our residents to the mental health program to help set priorities and explore potential new services.
